Executive Director, Alameda County Care Alliance

Cynthia Carter Perrilliat is the co-founder and executive director of the Alameda County Care Alliance, which created the Advanced Illness Care Program — a faith-based, community-led model in which trained care navigators from Black churches support people with serious illness and their caregivers across spiritual, practical and medical needs.
The model has been studied and replicated nationally as an approach to reducing disparities in advanced illness care, and she speaks widely on the role of faith communities in end-of-life support.
